Living Positive, also known as the Early Intervention Program, is specifically designed for HIV+ individuals, their partners and family members who want to take a pro-active role in their health care. The goal is to help individuals maintain a high level of health and to minimize the risk of further HIV transmission.
THIS PROGRAM CONTAINS THE FOLLOWING COMPONENTS:
Medical Evaluation/Lab Tests
Two comprehensive medical evaluations with laboratory work will be provided through a participating physician to monitor HIV status.
Case Management
ASN's Health Counselor is the Case Manager for this program. He will help assess needs, provide information on the services offered through the Early Intervention Program, and coordinate the necessary services.
Risk Reduction Education
The Case Manager will help assess behaviors which may put the client and others at risk for transmission of HIV, and help them see other options that may minimize those risks.
Health Education
The Case Manager will help clients look at choices the client is making that may influence their health in the coming years.
Emotional Support
Twice yearly, Living Positive clients can see a counselor to assess their psycho-social status. Referrals to counselors and to ASN support groups are available if there are issues that can be resolved.
